No, it is not fake. The little red box in front of him is a so called looper (maybe a Boss RC3). A looper can record a loop, instantly play back the recorded loop and you can overdub to the loop. So he plays a melodie and puts a second melodie in top of it. Pay attention to 0:42 where he makes the drum sounds by knocking on his violin. To work with a looper and make it sound this awesome you have to have good playing skills and a even better timing.
