Why doesn't the engine stall when most of the cab goes underwater? And if it does, why would it start up again when it was clear? I would think the water in the engine would prevent the necessary air flow. Any thoughts?

all you need is to keep air flow in the carburator...you can do that with a long waterproof tube that goes on the top of the truck cabin... (sorry for my english... but i think you got the idea)

Also, most trucks like this have exhaust stacks. Not a traditional exhaust system running underneath it. @ 1:55 you can see the exhaust pipe running up the back of the cab. As for the intake system it obviously had to be getting the air from somewhere (such as a snorkel) because if water was to enter the combustion chamber it would seize the whole engine as water can not be compressed.
