I'm pretty sure this dude died doing a jump off a cliff with some ropes that are meant to stop you near the bottom but they failed :(

  You are absolutely correct -- it is the same guy. There was an article about his fall and his career in Outside magazine about 10 years ago. He wasn't jumping when he fell, though. He was rappelling, and for some reason the knots were not tied in the bottom (he knew the ropes were only going to reach half way to the bottom -- the plan was to stop and re-tie them half way down, then finish the rappel. For some reason, he didn't stop himself and went right off.)
